We try to accept as many insurance plans as possible so it will be easy for you to get the eye care you need. Some of the plans we accept include:

We accept many other plans, too, so if you don't see yours on the list, please call us at (435) 649-5200 to check.

We accept cash, check or debit/credit card. Credit cards accepted are:

Co-payments are due at the time of your examination.

About Our Fees

The two most common fees are:

Healthy Eye Check-Up/ Eyeglass examination: $83.00
Healthy Eye Check-Up/ Eyeglass examination/ Contact lens examination: $130.00-$170.00 depending on your prescription

Rarely, during an examination, we'll find problems that require special tests. If so, we'll discuss them with you and let you know of any extra fees before performing them.  We strive to make quality eye care as affordable and easy as possible for you!
